The Challenge:

Half of those over 60 and two thirds of those over 70 have clinically significant hearing loss. Those with untreated hearing loss are at higher risk for falls, dementia, and depression, which significantly increase cost of care.


Only 1/3 of those that need hearing care seek treatment and on average people wait 5-years before pursuing care.  Stigmas, fears and lack of awareness of treatment options are additional barriers to seeking care for hearing loss and these closely related conditions.

Care Coordination:

Because chronic conditions often make annual wellness visits lengthy and complex, many seniors do not receive annual screenings for hearing loss, fall risk, Mild Cognitive Impairment (MCI), or loneliness/depression.  


Coordinating care across these closely related conditions is often difficult for large providers as staff quickly move from patient to patient. Many providers do not have the resources to develop specialty programs or to keep lists of recommended resources current.

As a patient, it can be confusing to know how to navigate the array of provider services or in understanding what options exist.  


Songbird allows the Provider to leverage a team and technology that is focused on providing best in class care, promoting higher engagement, patient satisfaction and better patient outcomes. Navigation is particularly important to Provider systems participating in Value-Based Care models.

Primary care physicians often do not have as much time tas they would like to spend with each patient. Additionally, building and maintaining current information about treatment options for many of these related conditions is challenging without support. 


Songbird is a resource to save the PCP time and strengthen the relationship with patient. The PCP can be assured that the patient will receive trusted, research-backed information and guidance on programs, increasing patient access to a variety of relevant services. Songbird can save the PCP time, due diligence, coordination, and patient engagement.
